To Whom It May Concern:

 

I hereby indicate my intention not to be bound by the prepaid entertainment contract between myself and <Company Name>.  Please consider this letter as my written notice of cancellation as stipulated under North Carolina Statues Section 66-121.

 

As the price of my contract was $XXXX and exceeds one thousand five hundred dollars ($1,500), I additionally request a Refund of the pro rata cost of any unused services under all contracts be submitted to me within 30 days as provided under North Carolina Statues Section 66-120 (3).

 

Attached you will find documentation that I have been provided with XXXX sessions out of the total 48 sessions that the prepaid contract offers.  The contract lists the price of each session $40.  Thus I have calculated the value of the pro rata cost of unused services as the total value of the contract ($XXXX) less the value of services rendered ($XXXX), or $XXXX.

 

I understand that by canceling this contract early, I am liable under North Carolina Statues Section 66-120 (3) for a cancellation fee of not more than 25 percent (25%) of the pro rata cost of unused services on all contracts, not to exceed five hundred dollars ($500.00).  As the pro rata cost of unused services is ($XXXX) I have calculated that 25% fee at $XXXX.

 

Attached you will find documentation that I thus far have prepaid $1,319 on this contract.  However, as I have only used $XXXX of services, I am entitled to a refund of $XXXX.  Less the early cancelation fee of $XXXX, I have calculated the final amount owed to me by BF-SOUTHEAST, LLC. as $XXXX.

 

Therefore, I hereby request that <Company Name> remit payment of $XXXX to me at the address in the heading of this letter no later than 30 days after the receipt of this notice in compliance with North Carolina Statues Section 66-120 (3).
